This structure will appear spontaneously if the region is adjacent to a hostile non-Slav region. It can be manually upgraded to a Kremlin, a more powerful and versatile structure. Should you not do that once the population reaches 15, the Detinet will be removed for good. A detinet is the fortified part of a Rus' inner city. It was a fairly common layout around the Xth century, as the vast tracks of lands that made Rus' were far from being safe, with principalities having only a firm hold on the most populated areas, with loose control on thousands of square kilometers around these urban centers. Pagan natives were still numerous, often hostiles and nearby principalities were not always friendly either.
